Hello everyone,


I have been having some very worrisome symptoms and I'm having trouble holding it together. I'm a 32 year old male living in the US.

-august 2nd Muscle twitching started in my right bicep, the same muscle would visibly twitch like a heart beat.

-middle of August I see my PCP I began feeling some weakness in my right arm. the muscle twitching was continuing in my bicep, but sometimes move to other parts of my arm/shoulder.

-PCP prescribed lexapro to see if it helped with the twitching. It didn't.

-Sept 8th I see a neurologist, he does some physical tests, finds nothing signifigant. and orders some blood tests for tremors( at this point I thought the twitching was tremors, jt wasn't). Blood work all comes back normal.

-Sept 25th continuing to get week so I go back to my pcp would orders and mri of my neck and shoulder, both come back normal

-October 10th I'm continuing to get weaker in my arm but not clinical weakness according to my neurologist. My whole arm would ache, especially after walking my dog if he was pulling. He suggests seeing about physical therapy.

-November 1st my arm is fatiguing very easily with barely any activity. My neurologist schedules an emg for 2 weeks.

-November 15th I have the emg, it's just of upper extremities. He stated there was some signs of neuropathy in my wrist that could be from carpal tunnel but nothing that indicated ALS. I told him I felt like my muscles on my right were getting smaller, he didn't see it. He did a blood test for various muscle diseases, not sure on the exact tests, but everything came back negative.

-November 16th immediately following the emg, I began having tons of faciculations in my legs, my neurologist said since the emg of my arms came back okay, it wasn't necessary to test my legs at this time.

-December I feel my right hand/fingers getting more clumsy. My right ankle also begins to crack very often and now it hurts after going on walks that I used to do every day. I noticed some bruising on my calf but figured I mustive injured it somehow. I continue having pain in my right leg, especially at my ankle, behind my knee, and near my hip. My right leg also feels very heavy, I'm almost limping at this point. I also notice im having some shortness of breath. I make an appointment to see my neurologist on January 2nd.

-December 24th I go to the ERI when notice I can no longer lift my pinky finger without also picking up my ring finger. Basically cant do this anymore 🤘. I'm also noticing my hand continue to get clumsy. My wrist and all my fingers seem to crack very easily and I'm having pain in my joints. They do an CT and mri of my brain and both come back with nothing significant. They refer me to one of their neurologist but they can't get me in until January.

-December 28th I am noticing atrophy in my hand near my knuckle on my ring finger, and on my forearm. I end up asking my neurology office if they had anyone I could see and talk to because I am freaking out. One of their PA's is able to talk to me and she notices no signs of clinical weakness. The reflexes in both my legs were +3. I showed her the atrophy in my hand and wrist she agreed that it was there. She then referred me to a neuromuscular specialist, I have that appointment January 25th. I have an emg of my lower extremities January 9th.
She also is trying to schedule another mri of my brain and an mri of my spine. I asked for another emg of my arms and she said they want to wait 3 months from my last one.

At this point my leg feels so heavy, I can barely walk my dog for 15 minutes. I feel my right foot is not nearly as stable as it was. I feel pain in my ankles, knees, and thighs for hours after walking. I feel some sensory loss in my right hand near my thumb. I'm still twitching in both legs, but not nearly as much. I've lost a lot of weight in my waist but it could just be from being so stressed over this.

I continue to have weakness in my right arm, although it is not clinically weak at this point.

I can still do about 35 push ups and 20 squats, although I'm way out of breath after.

I know I need to wait for a diagnosis but I feel so certain that I have ALS. My whole family and loved ones are trying to help me stay positive but it's very difficult. Especially with the way I can no longer 🤘. I am sick to my stomach. This progression has happened so fast. I don't have the results of my first emg but I'm going to request it from my doctor on Tuesday to update you all.

If anyone has any insight on this it would be greatly appreciated.

You've posted a lot... we really need to read the Summary/Conclusion of your last EMG.

You asked for another EMG of your arms and she said they want to wait 3 months from
my last one. That pretty much says she doesn't seem overly concerned as for ALS.

Once again, ALS is a rare disease and being just 32 years old takes it to even being
more rare.

Please ask for help with your considerable Health Anxiety.
 
Hey thanks for the reply, I'm actually on some anti anxiety meds to help with the health anxiety. Its still been very difficult.

as far as the out of breath thing goes, sorry about that, my statement was misleading. I took my dog for 2 40 minute walks yesterday. But today I could barely do 15 minutes because it's hurting my legs.

I will get the results of my emg posted as soon as possible.
 
Legs hurting from walking ( or hurting in general) is not an ALS symptom. ALS is not about feeling it is a primarily motor disease and you have a lot of sensory symptoms as well as a normal emg.
 
Sounds systemic, maybe post-viral, if anything. I would return to your PCP for labs and to baseline/track your strength and flexibility and run any appropriate labs, including inflammatory markers. If they are not up for further diagnostic exploration, I'd find someone else who is.
