Pathways Hospice has provided services to Bay Area families for more than 29 years. Formerly known as MidPeninsula, Pathways is affiliated with El Camino and Sequoia Hospitals. Pathways is a community-based, not-for-profit organization with one mission: to deliver compassionate, quality health care in your place of residence that promotes comfort, independence and dignity. An individualized health care plan is developed for every patient – under direction of their physician – with consideration to both health and personal needs. Services are available seven days a week. Nurses are on call 24-hours-a-day for consultation. Pathways Hospice is a state licensed Medicare and Medi-Cal certified hospice providing specialized, compassionate palliative care and support at the end of life. Thoughtfully orchestrated by the team of medical director, nurse, pharmacist, social worker, volunteer, home health aide and spiritual care counselor, care may be provided in a private residence, skilled nursing facility or residential care facility. Services include individual and group grief support and care is provided in the California counties of Alameda, San Mateo, Santa Clara, San Francisco, and Western Contra Costa.
