Carefor Hospice Cornwall is a professional hospice and respite care facility located in Cornwall, Ontario.
Mission
The Hospice mission is to assist patients in living as fully as possible while maintaining their dignity and comfort and to support family and friends. We provide compassionate end-of-life care for individuals who prefer not to be in a hospital and can no longer stay at home. Liason with home and family physicians will be maintained. Hospice Cornwall services are totally non-denominational and available free of charge to all people regardless of age gender race or religion. Professional care is provided 24 hours a day7 days a week. Families and caregivers are encouraged and welcomed to participate in the life and care of the patient at Hospice Cornwall.
Admisson to Hospice depends on many factors includingif the client lives alone family support how well they are being managed at home how well the family is coping how much emotional psychosocial needs are in place how well the client is coping and what their needs and requests are.
Referral criteria includes a PPS ofless than or equal to 40.
Hospice palliative care supports people living with life-threatening illnesses. It provides care for the needs of patients and their loved ones whether medical psychological social spiritual or practical. Physicans nurses physio/occupational therapists and volunteers work together with friends and family to ease a difficult life passage. The Hospice is home when home is no longer possible or practical making the last days of a persons life together with their family peaceful comfortable and meaningful.
