Services are provided in English. The program/service currently serves all ages. The program/service provides support and/or care for Cancer, Cardiovascular Disease, Infectious Diseases (i.e. HIV/AIDS, Hepatitis-C), Neurological Disorders, Renal Disease, Respiratory Disease, Care for all conditions, Anticipatory Grief and Bereavement, and Emotional/Spiritual Support.

Community Care Northumberland Hospice is a professional hospice and respite care facility located in Cobourg, Ontario.

Community Care Northumberland's Hospice Palliative Care Program provides an accredited and trained volunteer visiting hospice program from within a multi-service and community support organization. It provides compassionate and dignified care to people of all ages living with a life-threatening illness in need end of life care and supoort to caregivers.Service and support can be provided in various settings. Services include individual palliative visiting support bereavement and one-time occasion sessions caregiver and family respite vigil support group support memory boxes library resources education training and information. Service provides in home assessments and care planning case coordination community resource and referral addressing client need.

A person must be diagnosed with a life-threatning/limiting illness or requires end of life support. There are no age restrictions. Any individual family or friend who is experiencing or has experienced loss and is in need of support. Generally prognosis less than 12 months Palliative Performance Scale-PPS less than 50. There are no limitations on illnesses.

How would you answer the following question if asked by patient client resident and/or familyWe offer trained volunteers to go where the person is. If we start with the person at home we will continue to be by their side until death and provide a presence in the home. We will also provide support to family memebers. We offer a safe environment as our trained staff requires regular police checks.
