Hi all. 42/female here. I started twitching about a month ago. I climbed into bed and I was twitching everywhere all of a sudden. Over the last four weeks it has settled about 90% in my left leg ?butt cheek, all over thigh, knee, calf, foot) the remaining 10% are in my abdomen (usually lower area) and right leg. My left calf feels tight. I also experience little zaps. I have noticed low back pain and some knee pain.
I have had a clean neuro exam, a clean EMG/NCS that only showed carpal tunnel in the left arm, clear MRI of cervical neck, lumbar back and brain as well as all labs within NL.
This is wearing on me mentally and I am hardly functioning at this point. Started on Trintellix and have Xanax for when really needed.
I feel like I am being dismissed by my doctor and feel isolated and alone. The twitches are interfering with my sleep and are worse when I relax or lay down.

That points away from ALS.

You also experienced little zaps. You have noticed low back pain and some knee pain.
That also doesn’t fit with ALS.

You’ve had a clean neuro exam, a clean EMG/NCS that only showed carpal tunnel

What great news.

You feel like you’re being dismissed by your doctor. Why? Read the paragraph just above.

You’ve been prescribed Trintellix and have Xanax for when really needed.
The doctor saw you are dealing with considerable Health Anxiety.

I hope your doctor can help you, assure you… so much points away from ALS
 
You could ask about screening and if positive medication for Restless Leg Syndrome, especially if the twitches improve when you start moving. A sleep study and/or tracking "movement" in sleep with a wearable could be revealing.
