I have composed a mini questionnaire on fasciculations to help people who are anxious about their symptoms. In the bottom are a few questions on fasciculations for people to answer who have already been clinically diagnosed with ALS. Your input is greatly appreciated. Hopefully this will help people in general.

1. Have you noticed a specific sequence of fasciculations, atrophy and cramps. Or are they random?
2. What was your chief complaint to doctor's visit? i.e., fasciculations, weakness, falls.
3. Are fasciculations generalized or focal?
4. If generalized, do fasciculations jump around or continguous?
5. What is the consistency of fasciculations? Be specific as possible
6. Once you notice fasciculations in a specific spot, have you notice that they come and go? Be specific in terms of days, hrs or weeks if you can.
7. If any other questions are relevant please feel free to input.
 
Rich - if you go to the forum titled General Discussion (that is for diagnosed ALS patients), there was a questionnaire posted similar to yours not too long ago. Some of the PALS answered it, but others said their story would take far too much time. Maybe if you do a search for it you will find the answers you are looking for. It was posted by Jamie and he did it for the same reason, to help people with questions. Good luck. Leslie
