i started with eye twitching in last Nov and then spread to all my body like back/butt/calf/foot/arm/toe/thigh/hamstring

EMG was clean in Jan.

After Jan, the twitching is getting worst and more consistent.
I noticed that twitching is worse at night. sometime i felt my neck/abs ripple also.
No weakness yet. just sometime back and arm sore.

Also i felt that sometime during night, my big toe felt numb.

Should i worry about ALS onset because the twitching on my trunk and wide spread.

If you could read here: Read Before Posting. It explains why sensory issues and twitching absent clinical weakness points away from ALS and towards something else. It also recommends you visit a doctor first, as they can provide you direct answers about any concerns you might have.

Any PALS here have whole body twitching before weakness and get the DX later?
Any PALS here have first clean EMG and then DX later with dirty EMG and what's the normal lag between the first one and second one?

To your questions, I don't see any way that they even apply to you, let alone others here. Whole-body twitching argues against ALS regardless of timing, and even more without any impairment in function.

My best advice is to act like you never heard of ALS and figure out what makes you feel better. Some magnesium lotion on the hot spots, paying more attention to food, liquids, electrolytes, stress, sleep, exercise, etc. If your toe is numb, a good place to start is your bed, wherever you sit during the day, and your shoes.
 
I have two questions

1) I had whole body twitching for 6 months now. I noticed that during night, the twitching is getting worse and wake me up at night.
Any one experience the same? my EMG was clear 2 months after twitching started and it was clean. Since then the twitching is getting worse.
2) Also i noticed that when i half reflex my calf/thigh muscle, there is rippling/waving under the skin, is it normal or indication of ALS?

No, the EMG was not too early. You might want to get a sleep study to see if you have a movement disorder at night that could be affecting your day.

Only a doctor in person can evaluate what's happening with your muscles. Why don't you show someone? But it could easily be a normal contraction from what you say, especially if you are resisting gravity from your position.
