We figured out, dealing with my stomach pain from feeding tube insertion:

When I slide down in the hospital bed:
1. flatten the head of the bed.
2. grab a wrinkle or edge of sheet on each side.
3. bend knees.
4. push feet down while pulling on the sheet edges, do this multiple times as needed.
5. when satisfied, raise head of bed again.

To prepare for standing from bed to get to commode at foot of bed:
1. use above techniques to scootch over to edge of bed, and correct any sliding down.
2. also rotate a bit, to get slightly perpendicular to edge of bed.
3. raise head of bed more than usual.
4. very slowly, I lower one leg till foot is touching carpet.
5. same, with second leg. Assistant can do this if he does it in slow motion.
6. depending on pain level, may be able to do a partial, modified log roll.
7. avoid a sit-up as such. Instead, raise the upper body through a sideways motion.
8. my very strong son can help with this by putting his forearms under my underarms.
9. now scooch down towards the foot of the bed -- this hurts less than walking/shuffling.
10. when standing and moving sideways or turning, assistant can stabilize me with hands on my waist, or supporting both hands or arms -- the key is for me to control my movement -- he just had to be an immovable tree trunk. If he controls my movement, it hurts.
11. adjust height of commode to maximum, and make liberal use of grabbing the vertical part of the bed at the foot.
12. keep a big book under the edge of the bed. When sitting on the commode, inch the book forward with my feet, so I have a foot rest. Dangling feet are bad.

By following all these steps, it is getting less stressful.
