Use Google Docs to transcribe/type your spoken word! It has been very accurate for me. Many verbal grammar commands as well. Saves doc automatically.

I've been using Google Docs for a couple weeks to quite literally transcribe my spoken word into a typed document before my very eyes. There are quite a few verbal grammar commands as well, but you can also make changes and corrections of text and grammar manually. I have a Rode microphone on a boom arm attached to my desk, and my enunciation is still fairly good. I don't know what lower quality equipment would do. The resulting document is automatically saved at all times. It can be saved in multiple formats, including Word, and printed. I'm dictating an oral book of my life's story, which can be edited by me while I still can move limbs. I can then read the resulting manuscript into an audio book. I've also purchased an ergonomic keyboard and mouse.
 
Thanks for the reminder, Megatron. All the desktop and mobile OS have built-in dictation now, and there are many cloud-based apps that add functionality, such as Otter, which enables you to place the cursor at a point in the transcription and hear the audio, making error correction easier. You can also import audio files from the past to transcribe them.

I have found that the built-in mike on many modern computers works better than external mikes, depending on your dictation style and setting, so I'd try what you already have before buying anything.

For some PALS, mixing up dictation with typing may be easier, like taking breaks when you get tired, and that is possible as well.

Finally, I will put in a periodic mention of the ErgoRest (and other results that you get when you search on that) line of arm supports on Amazon, which may extend your keyboarding days.
 
I have been using Dragon Pro for a month or so for work. Does ok, decent support and is native to machine. A little pricey.
