This is a joint request from the ALS Society of Ontario and the ALS Society of Canada.
Here are the survey details:
"Alec Muir, whose mother has ALS, is studying at Sheridan College in Brampton and is doing a customer satisfaction project on ALS for one of his classes. As part of this project he is doing a small survey of people living with ALS and their families. Could you arrange to put up a link on ALS Canada website? He needs about 50 responses from families by July 15th."
