I also have bulbar-onset ALS. I have use of my legs and my right arm is still strong. I have s feeding tube, but going under anesthesia to replace the g tube with s a j tube since I’m getting a lot of mucus in my throat, that is constantly choking me. I have reflux very bad. What does s person do to get rid of the mucus so you don’t choke to death?
 
Do you have a suction machine? I have mucus secretions plus excessive saliva. I take Mucenix twice a day and use my suction machine as needed.

Some suction machine users use a smaller tip (dental) than I think is sometimes provided with the machine.

I would get a second opinion on the tube replacement since I am not seeing evidence that a J tube will reduce mucus in your throat, nor have I ever heard that. And J tubes have their own downsides.
