If this is the first thing on your mind when you wake up >4 mos in, I'd really consider counseling. There's no point to ruining your life with this, and you acknowledge that the anxiety itself affects your speech.
Often there are issues that underlie the anxiety about something that seemingly isn't real to anyone in the field. It can be hard for you to see them when you are in the weeds. There is no shame with trying a visit and seeing how far you can get.
I would also explore the physical approaches I mentioned in a post above, plus therapeutic massage for your neck. Cover all the bases, and perhaps you will feel less worried off the top.
