Hello I have been experiencing bouts of weeknesd twitching and shortness of breath. I was feeling weak in limbs and neck in 2023 and in August had an emg lower and upper extremities came back normal accept for carpel tunnel . Continued on with life and over time I began to feel weak again this time I noted the abdomal twitches and shortness of breath so in August of this year neuro gave me emg of left sides lower and upper e extremities . I’ve also had multiple physical exams morning was doing and have 5s on everything . My twitching has persisted in stomach/diphram area real bad and shortness of breath have progressed . My question is if I have respiratory or Bulbar onset would if have been caught in the emgs and/or physical exam since those muscles weren’t tested

It is highly unusual for an emg done in those muscles to be completely normal if you had bulbar and respiratory als. They didn’t directly test those muscles but there are usually findings as the emg is usually abnormal in places you feel fine.

An exam should detect bulbar so if that was fine multiple times I wouldn’t worry

I sincerely doubt you have respiratory onset ALS but if you are having breathing issues I would ask about pulmonary function testing. That can rule out respiratory als but more importantly can see if you have another cause for your symptoms

We often see people report limb symptoms and get ruled out as you did then go on to worry about bulbar and or respiratory als.
