Searchlight Support renewal for help with Radicava

Just wondered how I go about renewing for 2022 with Searchlight Support? They sent something in the mail which I can't find. Their website isn't really clear. I went and did a docusign and signed it with today date.
Unless I have to fill everything out all over again like an application. Everything is still the same info they had when I signed up several months ago.
I can't speak so I can't call to find out.

From their [not the greatest] site:
You must re-enroll to remain in the Program in 2022. Verification of your insurance benefits is required to confirm that you continue to meet the eligibility requirements. To re-enroll, you must complete the Out-of-Pocket Assistance Program Enrollment Form [I attached this to the post] and fax to 1-888-782-6157 or submit to Searchlight Support®, PO Box 2930, Phoenix, AZ 85062.

I had received a letter for renewal but didn't have to do anything. My neurologist took care of everything so I had no interruption on infusions.

I don't know how to know if it is automatically taken care of
