I've had muscle twitches for 6 months-- since I had COVID. I was very scared in the beginning but at that time, they were all over my body, no noticeable pattern which made me feel better. They have decreased in that 6 mo (in the beginning it was almost constant/everywhere), but now they're mostly in my legs and more often in my left leg, which i find worrisome. I guess I also have weakness-- my legs are shaky, which I notice sometimes and the neurologist pointed out yesterday. they don't FEEL weak, but the tremors would indicate that they are. So what sent me over the edge was at my neurologist appt., yesterday, my reflexes were brisk or "jumpy" as he stated. I'm not sure if that was bilateral or unilateral. The combination of these three things makes me really, really scared.

Mitigating circumstances are that the twitching started after getting (mild) covid, and I also have a brain herniation called Chiari malformation, but my herniation is only mild and it wouldn't explain all of this. i'm 36, two little kids, very frightened. At this point he only ordered an MRI to see what's going on with the chiari malformation/ if it's worsened, but he said he was concerned and I got the sense it was about more than just that.

I hope you get some answers soon.
It certainly doesn't sound like ALS - is your neurologist a neuromuscular specialist?
If they were concerned it really might have a chance of being ALS then an EMG would have been ordered or you sent straight to a neuromuscular specialist. So hard as it is, try to stay calm and follow the process.
Twitching, tremors and a few jumpy reflexes are not what we call a red flag for ALS. Let us know how the MRI goes.
