Hi all,

I will try to make this quick as don’t want to interrupt anyone’s holidays and want to get on with my own, but am a little confused and would like some clarification if possible.
I am a 25 year old male.

I’ve had muscle twitching since middle of October. It started in just one muscle in my upper right chest almost constant, and has since become sporadic in any and all muscles in my body. I have also had cramps similarly in any and all muscles. I have no issue doing regular tasks although it does at times feels odd or different. On top of that my left hand ring finger has started to “lean” and when resting my fingers my ring finger no longer rests straight it now leans on top of my middle finger at rest
Recently I’ve also had trouble swallowing, I do not choke but it feels like something is stuck in my upper throat when I do eat. Also I’ve noticed chewing to take longer and my mouth gets tired very quickly.
I have been to a neuromuscular specialist who said I had a normal clinical exam only noting bilateral Hoffman’s signs. He also did an emg of all 4 limbs and it was totally normal, minus finding mild bilateral carpal tunnel.

I have been having headaches, dizziness, painfull body wide as well and digestive issues. I’ve recently had an MRI of my brain and am waiting for results. I am also Vitamin D deficient.
I know those last few symptoms point far away from als and after my clean emg I had my fears relieved. But with the swallowing issues and the finger thing I mentioned I just wanted to confirm. Twitching, cramping and muscle loss/death would show on an emg if it was caused by LMN signs from als correct ? so basically I’m asking is although emg does not show UMN signs, because I am having “Lower” signs, those would appear on my emg and because the emg is clean, those symptoms are not from als.

I would also like to add that my biceps and calf’s at times feel very tight, it’s not after or during activity and it’s not constant. Comes and goes.

You don’t describe ALS at all. And with normal clinical exam and normal EMG, you’re officially cleared. Yes, twitching and cramping are lower motor neuron signs in a person with ALS and would have shown up on the EMG if you had ALS.
You mention concern about PLS. People who have that show spasticity on exam, and the neurologist would have pointed that out to you.
