Cervical compressive myelopathy could cause extensor weakness in the arms and flexor weakness in the legs and long tract signs. As it's been excluded, along with neoplastic spinal cord compression something else is probably causing the symptoms.

I assume you're dealing with a peripheral axonal polyneuropathy, considering CNS imagining was normal? EMG findings in motor polyneuropathies might show acute denervation and MUAPs changes, especially in acute and progressive processes.

My point here: if the symptoms were only due to his neuropathy, EMG would not have shown acute-chronic denervation in other innervation sites with normal NCS studies (excluding MMN as well). This points to LMND. His neuropathy, however, might contribute to aggravate the symptoms.

If UMND is unclear, but LMND is, then you might consider PMA. SMA doesn't seem to fit the profile. Please, go see a neuromuscular specialist!
