Having a pulse that is frequently 90+ is another good argument that his body is working too hard and a daily BP med would help bring that down. He can't feel that great with a pulse running high all the time. BP of 90/53 is exactly what is chasing the pulse up, among other things. The heart tries to compensate for the low blood flow. Anyway, the idea is to even up.

The nurse obviously knows nothing about the release profile of ACE inhibitors, if she is acting like they are a prn med.

Your internist can help with this -- you do not need to deal with hospice on this.

A family practitioner, general practitioner, nurse practitioner, physician's assistant, internist are all primary care clinicians who can write for something like this (well, some states restrict NPs and PAs, but they can get a rx signed in their group). Every PALS should keep up a relationship with one for moments like this...
 
Thanks Laurie, we have a PCP that we have a good relationship with. I will check with him.
