I went to my Primary Care doctor yesterday due to migratory leg fasciculation for 2 months. Upon his neuro examination, I scored a 5/5 on strength, however he noted my left leg is strong than my right, but he scored me a 3+ on reflexes and diagnosed me with hyperreflexation, I kicked my legs far out on the knee hammer test.

Yesterday I decided to do calf raises and now my right calf is sore. I’m still able to do everything physically, but the twitching and hyperreflexation has me very concerned. Based on this, how concerned does this sound?

Have a read here: Read Before Posting for answers to the common questions that bring people here. Twitching absent clinical weakness can mean so many different things, so you would have to keep working with your doctor to figure things out. Your thread title mentions an EMG, so I am assuming you are getting one?
 
Thanks for the reply. Yes, I’ve read that. What concerns me is that I’m hyper reflexive in my legs too.

Yes. The my PC Dr made the referral.

Hyper reflexes can be a normal baseline for some people. From the above linked post:

Hyperreflexia
Hyperreflexia, or overactive reflexes, cannot be judged by you. This determination must be made by your PCP or neurologist. Hyperreflexia is not uncommon and, especially if it is bilateral, is often meaningless. Speaking of reflexes, Hoffman’s sign is found in a number of healthy people so is not necessarily worrisome either.
