My FVC numbers have been on a steep decline since november. I feel breathless often now. Whenever I try to blow out hard in the test it feels as if a valve in the base of my throat closes shut and cuts off all my air-almost like a laryngeal spasm. Does this ever improve or level off? if not, I think my days are numbered.

I have had the same problem with throat closure on my first test. After using cough assist before the next test, my numbers improved a little. Now they're down again, and the throat closure is back. I don't use the Cough assist near enough, I guess.

The glottis is like a valve and once it's not opening and closing normally, likely to remain so. Neil, you don't have to do keep doing FVCs. Just say no.

One of the things BiPAP tries to do is keep you from needing to exhale that deeply (with the EPAP, Ti duration range and cycle settings). If you still feel like you are working too hard to exhale just in normally using the BiPAP, I would adjust your settings.
