Depression is co-morbid with ALS and frequently affects both PALS and CALS. I found EMDR therapy to be the most effective treatment for depression. It's normally used for PTSD.
May not work for everyone, but it worked so well for me I thought I'd recommend it. I did it a year ago.
EMDR is Eye Movement Desensitization and Reprocessing, a weird name for a somewhat unusual therapy. It seems akin to hypnosis, but I was told by a hypnotist that it is NOT hypnosis.
After a year of sullen, irritable, angry, and numb depression (on anti-depressants) I changed therapists and inquired about EMDR, which is used to treat PTSD. We performed a half-dozen sessions of EMDR and I have been much more happy and productive in the year since we did it. In fact, I no longer see the therapist at all, the EMDR did the trick.
In short, those of you who have been suffering from depression might try to find a therapist who practices EMDR. It worked really well for me after my wife died.
