Hello, I am a 32-year-old man. For more than two months, I have been experiencing a tingling sensation in both calves. Later, I started having fasciculations in both legs, followed by my arms and shoulders. I also notice that the fasciculations are becoming more frequent over time.

Another symptom I have is that both of my arms get tired more easily, but I can still lift 40-pound dumbbells. I already had blood tests and a head CT scan done. Everything came back normal, except that I was low in vitamin D. My doctor performed a physical exam, which was normal, but she didn’t test my reflexes. She didn’t notice anything unusual, but based on what I told her, she referred me to a neurologist.

While researching online, I came across someone my age who was diagnosed with ALS. They started with fasciculations all over their body and later developed weakness in their shoulder.

Here are my questions:
1. If ALS starts with fasciculations, how long does it typically take before weakness begins?
2. Can vitamin D deficiency cause fasciculations?
3. If I start taking vitamin D supplements, how long should it take to see results, assuming this is the issue?
4. My appointment isn’t until February 14—should I try to get an earlier consultation?
5. Do my symptoms sound like those of someone with ALS?

1 this is rare. Especially at your age the cases that truly have twitches before weakness are almost always genetic. It is common that people who think they twitched first turn out to have clinical weakness when they go to the doctor. And usually weakness follows very soon. For almost everyone though it is failure, then twitching

2 yes

3 symptoms usually persist for a while even after your blood work has normalized. Be patient

4 no I wouldn’t. 2 months is actually amazingly quick
