Hi there

I have been experiencing muscle twitching and tongue atrophy for 5 months now.

My muscle weakness is primarily proximal and bulbar. I have seen a general neurologist and a NM specialist and have had 2 clinicals. I have 3+ reflexes bilaterally and no reaction to babinski.

I have had 2 EMGs 1 body and 1 tongue.
Only finding was fasicilations in all muscles.

My question is.
Can bulbar start as just atrophy as initial symptom?
Are breathing issues rare for early symptom?
If i had 2 EMG 2 month apart would this clear me of ALS?

Unlikely/yes/pretty much.

You say "weakness" -- was this clinically documented? What the tongue looks like is really not the issue in MND, and twitches are as you know very common and usually benign when widespread.

Unless your breathing has been shown deficient through testing, I am not sure why you are asking about it.

There are conditions that are cousins of ALS that are more confined to the bulbar region. I am sure you will be followed to make sure that this is not so, and that there is nothing else going on.
