Hi,

I like to know if it’s true that if a person with als who uses a bipap 24/7 will not need invasive ventilation?
 
That's not true.

With most pALS eventually invasive is required.
 
Well, to clarify:

Some PALS are happy and productive with a trach. Others are not.

Many PALS die within several years even if trached (invasive ventilation). The disease progresses even with a trach.

Most PALS choose not to get a trach.

Many PALS do not live more than a year or two once they reach the point of needing BiPAP 24/7, not just because their breathing gets worse, but often because they choose not to. Since every PALS is different, that choice might take place any time in the disease.

And on the other hand, in Japan, where society expects it, the trach rate is reportedly 70%
 
Yup, there are definitely cultural differences. However, the Japanese trach rate appears to be more like 29%, per this paper and a quick scan of more recent papers seemingly confirms that rate has stayed stable in recent years.
