Any advice very much appreciated!! My partner has been to see a neurologist and they are only really wanting to discuss the possibility of ALS and everything else they have said is not linked!

He has had many years of ear/sinus problems, which was put down to pressure in the inner ear!! He started with a hoarse voice/ not slurred but difficulty in pronouncing certain words, with no other symptoms maybe 12 months ago which has highlighted the possibility of ALS!

He has had a full work up of his reflexes/muscle strength/balance 3 months ago which were all fine, with no change! He is of a muscular build with no weakness or atrophy but the occasional random muscle twitches!

He has TMJ pain on both sides of his jaw and terrible sharp pain in the point under the ear which radiates into his neck/throat/back of tongue and up into his temple on palpation/yawing more so on one side and can cause tongue cramps…He also has a severe malocclusion which isn’t new! He has elevated thyroid levels which is no surprise as both his parents struggle tremendously with hyperthyroidism and he has arthritic spinal changes which showed up on the mri scan! Normal CK level!

He has been suffering with terrible stress with all this which has resulted in him having a globus feeling, that he just can’t shake off, causing more anxiety and not helping the hoarseness!

I feel that more investigation work must be done before a diagnosis of ALS is to be made but it seems that the neurologist has made his mind up and isn’t willing to undergo further investigation!
 
If you are saying an ALS dx has been made before an EMG, that is surely not appropriate and I would take that up through your GP for an alternative referral, or through the NHS.

Has he been fitted with a bite guard/been recommended braces or other therapy for the occlusion issues?

Has he seen a speech therapist to evaluate whether the speech difficulty is related to his teeth/jaw?

Is the hyperthyroidism being treated?

I agree that from what you have said, ALS is hardly top of the list.
