r.b. stated that he saw 4/48 fALS/sALS reversals, same ratio in reversals like in occurance. wtf? that brings completelly new image. between a lines, it means that no als is inherited, BUT JUST CONDITIONS FOR DEVELOPING ALS . further, it means that all (epi)genetic changes are just help to develop als.
comments?

I believe your statement that no ALS is inherited is false and , frankly, hurtful. No one knows why any of those 48 had reversals. For whatever reason their bodies were able to repair. that does not mean that other people doing the exact same things would get the same result

i just conclude it. if you inherit blue eyes, you can not change it. if you inherit als, you can not heal it/recover it. thats what 4 of 48 (app 8%, same ratio in occuring/reversal) did. how? nobody ever will find out. what can explain fals reversal??

sorry, instead (app 8%, same ratio in occuring/reversal) should be (app 8%, same ratio in fals/sals)

Genetics is complicated. Just because you carry a mutation does not guarantee that you will get a disease. Google the concepts "gene penetrance" and "gene expressivity". Data on penetrance for ALS genes is hard to find, probably because it is so complicated. But if you look at Huntington's disease, which is caused by a mutation on a single gene, you get the idea.

Here's an article that theorizes a multi-step process for ALS: The multistep hypothesis of ALS revisited.
It's a difficult article but my understanding is that for sporadic ALS to be triggered requires 6 contributing steps, C9Orf72 ALS requires 3 steps, SOD1 ALS requires 2 steps, etc. Steps that trigger ALS could be environmental toxins, physical trauma, viruses, etc. So if you have a SOD1 mutation it is likely that in your lifetime you will encounter two things that, in addition to the mutation, cause you to develop the disease. Conversely maybe you can somehow trigger a reversal? Just because a reversal occurs in fALS does not mean that ALS is not an inherited disease.

Sod1A4v ( most common US variant) is almost completely penetrant by 46. C9 by 80 though over half of carriers will be affected before 60.
if using step theory the thinking is that the initial several steps are things that are virtually unavoidable - that you have to experience in life). The other steps are probably the things that can be modified if we only knew what they were. The genetic studies look at lifestyle The Miami study in particular takes highly detailed information on intake and updates each visit.

some families can trace cases back over generations and continents which seems to indicate the initial triggers are not going to be easily modifiable.

Thanks for the info Nikki.
