    I have been diagnosed with bulbar ALS. I wake up every night gasping for air. My vocal cords are messed up and I can't tolerate the pressure from a ventilator. I can breath better when standing. What is the course for this? Do I need a trach with a ventilator. Is there anything I can do to prolong my breathing?

    You may also want to add information -- have you tried adjusting your settings on the machine, using a nasal mask, do you still speak, do you have a feeding tube, etc.
