Not sure my post went through on your thread. But as the daughter of someone just diagnosis with ALS in September I can really relate to your questions. In the short time since my Mom has been diagnosed ( and she is 77 and has bulbar onset so very different from your brother) I have been pushy about Lyme's, MG and looking for other things. Due to your brother's age, he would be great candidate, if it is ALS, for the promising new treatments. The deeper I go into the medical abyss the less I think they know about desease, its causes, triggers and contributing factors - so I encourage you to continue to help him through this.
