Before being diagnosed with ALS, I had a nerve biopsy which suggested I have/had CIDP. I have been receiving IVIG which does give me a boost. My doc said he has never heard of anyone having both conditions. Have you?

Nope, never heard of people having ALS/MND alongside with CIDP.
I might be wrong but I think ALS is a pure Motor Neuron degeneration which in turn affects the muscles (as the Motor Neurons that nourish those group of muscles die off) whereas CIDP is more a muscular degeneration and auto-immune.
Although, the symptoms on both diseases may look very similar at the beginning.

However, if you're responding relatively well to immuno-globulin that apparently would point away from ALS, in my opinion. Pure ALS would never respond to that.

If I was you, I would seek for a second or third opinion from another Neuro.
