Default Medication Help

Ok, Eddie is currently taking 24mg of dilaudid every 4hrs and 16mg every hour on the hour for breakthru pain. It is not working, he is in severe pain in his shoulder regions. I have tried explaining to hospice. All they do is keep increasing the dilaudid and it is not working. Help what other kind of med is available out there for him. I feel like we are reaching the end of what is available.

I take 3mls methadone three times a day. I take 30mls hydracodon for break-through pain. the reason he isn't getting relief is because his body is habituated to it. ask for something different. I tried several things before the methadone and I am very satisfied with my pain management
