Help with diagnosis

What criteria is used to differentiate between limb onset and flail leg. Is progression or lack of it the only measure?

As always, thanks for your help.
 
My understanding (and others please correct me if I’m wrong) is that flail leg is a primarily LMN disease involving one or both legs, starting distally, and with very slow progression. Lifespan and prognosis of this is better than average for ALS, but I believe most of these gradually evolve into ALS and acquire UMN findings over time.

What I don’t know is whether or not EMG in areas other than the affected leg(s) would show abnormalities right away. I suspect it would eventually as the disease progresses.
 
Thanks Karen. I think I posted my question in the wrong place. My husband has already been diagnosed and his progression is thankfully slow so I was just trying to gather some info before his next clinic visit.
