I had some good news yesterday. As some of you know, I have had slow progression since my ALS diagnosis in 2000. My neurologist has puzzled over it for some time and this week had me come in for another EMG and physical exam. He found a complete lack of fasiculations (not news to me) and very little muscle atrophy. Hallmarks of UMN involvement were there in spades; hyper-reflexia and severe spasticity. The EMG showed almost nothing in the way of LMN involvement. He did not perform the original EMG on which the ALS diagnosis was based. That was done at the Montreal Neurological Institute where minor LMN impairment was noted. I had not appreciated the significance of this at the time (if I even knew it). Based on the exam and EMG he feels it is more likely that I have PLS than ALS. Sounds OK to me.
