Caring Connections has quite a bit of information for Advanced Directives including printable documents by State, tips on how to talk to family and physicians about your wishes, support and resource information for caregivers, pain management and Hospice among other things. It is not ALS specific but has some good general information.
Note: It is sponsored by the National Hospice and Palliative Care Organization and has several End of Life sections so it may not be appropriate for all.
