Hi, I posted back in September: 72 year old woman with drop toot, numbness and weakness in both legs following blood lot and worse after spinal fusion. Problematic EMG. Neurologist
said "could be anything from Neuropathy to ALS."

I now have an evaluation scheduled at Vanderbilt with Dr. Peltier on Dec. 10 and I am busy trying to get my records together.

My question: There is a HUGE amount of material. Right now I have it divided it into medical specialties: neurology, neurosurgery, rheumatology, hematology, etc. But I wonder if I should pull out the most crucial info such as EMG, nerve conduction, blood work, MRI CDs and have a seperate file for these? I want to make the best use of my visit .

Any suggestions about how to organize these materials would be greatly appreciated. Thank you.
 
Please read over the sticky by Lgelb on the road to diagnosid if you have not yet. As organized as possible as you are doing sounds great. Are you hand carrying? I do recommend hand carrying but also getting them sent ahead. At my teaching hospital the fellow assigned to a patient will review and organize things ahead of time. I did organize the papers I brought with me with the most relevant abnormals on top. Your case is somewhat different as you have records from various specialties

I agree with pulling to the top things like ncs emg and Mri discs. If you are fortunate enough to have thorough consult notes from the specialists those might be useful at the top of each subsection

Thank you! Definitely planning on hand carrying. Should I send EVERYTHING ahead as well? Just the hospital record is over 400 pages! That is a lot for even a fellow to go through!
 
Can they be transmitted digitally? The thing is it will be hard to know what will be important to them. And the fellows like all doctors are used to mountains of information.

I would place materials in ascending date order, and agree, send digitally as well. And burn/bring a CD containing the digital file in case those are "lost in transmission." Vandy's a big place. If everything's scanned/PDF'ed, easy to add a hyperlinked ToC, which will help readers.

Write a one-page summary sheet (more info on that in the sticky Nikki mentions) for the top of the stack and for the digital file.

Thanks! I have read the sticky and already have done a one page summary. Now, to get some family help to send the info digitally! Will keep you posted!
