I found this while browsing communication devices.
I don't know if this is readily available, but it sure seems that it could help PALS that have the use of hands.
It is a sniff activated system that is used for communication, working on a computer, and even to control a power wheelchair.
The link has videos of its use. (the videos are QuickTime, so work on iPads)

I emailed the person that conducted the study to see if this was available for public use yet. Much less cumbersome, it seems, than eye-gaze systems, perhaps. I'll let you know if I hear back.
