I think it is saying that substances produced in the muscles may contribute to the death of motor neurons and axons. Whether this is the primary issue in some cases remains to be seen

Whatever is happening on a micro level ALS is still a motor neuron disease

I hope this clue proves helpful in finding the answers we all need
