This is a survey on computer use (PC, laptop or tablet) by pALS or their full-time caregivers.
It’s focus is Microsoft Windows but I need a complete picture, so if you use a different OS, such as iOS or Linux, please select the ‘Other’ Option:

The rest of the poll consists of combinations of your phase of progression: Early, Mid or Late as defined below.

- Early: Aware of ALS symptoms but fully capable
- Mid: Impacted noticeably, needing occasional to frequent assistance
- Late: Require assistance for most if not all needs

Combined with how frequently you OR your *full-time* caregiver use AND/OR have immediate access to a Windows computer as defined below.

- Frequent: If I'm awake, it's available
- Occasional: I can go without it for a few hours
- Seldom: I have one but seldom use it

Example: Per my definition, I’m Late stage, and use my PC part-time, so my option is -> Late / Occasional

Krissy was a veteran. VA issued her a Windows-based "eye-gaze, do everything and control anything" computer made by FRS. But frankly, she never had much use for any technology.

Mid/Frequent but also use Mac (am doing so now)
